I've had this issue occur for me and I fixed it by erasing the ubi partition on first boot and restarting.

@rany2 after booting to initramfs-kernel.bin, doing mtd erase -r firmware and then reinstaling / sysupgrading the system seems to be working. I'll do several additional reboots / tests to check that it doesn't break later on.

I think that's what I did. Sorry I forgot the details and I didn't have an OpenWRT wiki account at the time (so I wasn't able to update the docs) but I'm very sure that erasing one of the partitions fixed it for me.

(Edit: if I'm not mistaken I did this from stock AsusWRT not OpenWRT, but it should be the same)
