What's "and similar"?

If you shift everything but Patient Birth Date, the Patient's Age at Acquisition gets mucked up. Also, you risk having an infant's image but apparent age of 17.
As you know, we've been talking offline. Recalculating Patient's Age at Acquisition may be the way to go, especially when there are multiple studies for a single patient.
This is not the easiest thing to do, but not terrible.
I'd be inclined to use the DateTimeAnonymizer to actually do the date/time shifting, as it understands all kinds of attributes, including those for which DateTime is an infix!
The trick then is priming it with the right shift amount. I'd thought it would be tough to identify when we were shifting between instances, but Anonymizer.anonymize knows, so maybe that would be the place to calculate the new offset and maybe insert an appropriate handler at the top of the _element_handlers
