I was expecting some plugin to add watermark to blog images. For the first time loading, it took ages to load images from a post, and images were loaded with the watermark successfully. I have not checked the source code yet, but I guess it was creating new images that with watermarks. However, when I refreshed the page (actually any page), the watermark was gone forever. pretty wierd.

May be I misunderstand your logic but I change logic create figureElement so it can work in my case. Please take a look.
Image title will NOT generate if:

1. Setting is NOT enable
   OR
2. Image title is empty

So in ImageService.php I change line 161:
if(empty($this->s->get("enable_captions")) || empty($node->getAttribute("title")))
