update template's index.php with bootstrap3 classes, to replace bootstrap2 classes. Reference http://getbootstrap.com/migration/
And smaller tweaks to emc custom styles to fix any style conflicts.
Also, unlinked cdn in account's view so all views uses bootstrap3 files linked via template.
Remove class button from upgrade and inject and replace with btn so it uses bootstrap and not our custom styles.
