Setting up OpenMRS instance on AWS.

Clear out the demo form, and make it as close as it could be for final release (don't spend too much time on this, but fix the obvious issues)

The purpose of this task is to design a simple HGU data collection form for use as a trial/demo.

But of course, the form you build should be prettier, and look like the vitals form that we have on the OpenMRS demo right now!

Also note that not all the attributes in this form should be text fields, as displayed. For example, patient name, age, sex etc. are attributes, that are treated differently. Please refer to other forms for guidance :)

Using the existing vital form (and other forms as an example) build out a sketchy form that can be demonstrated to work on a standard OpenMRS server.

For this task, complete and end to end workflow of the form. I should be able to save a form, then retrieve and edit it. Don't worry about the view component just yet. Focus on the save/edit for this point.
