I also tried changing the options on event type, no luck.
I also used user form, exactly the same way you have mentioned in the document, No Luck.

Whatsapp notification is configured wrongly. You have mentioned the fields, which will be used in templates but I don' t see any field in your template. You need to create proper template and whatsapp notification.
