finalize Docker deployment

Add instructions, test, and put it up on dockerhub.

strange permissions issue with visus.config

For on-demand to work (one some systems), it seems that visus.config needs 777 permissions which is insecure. Need to understand why and more importantly, explain the proper setup in the INSTALL.md.

Generate xml using cdscan on the fly rather than as a preprocess

occasional connection issues with mod_visus

Technically this is a mod_visus issue, and could be related to docker or virtualbox, or even the systems that are connecting, but we are having occasional hangups when connecting using the following command:

curl -v http://155.98.20.215/mod_visus?action=list

The two errors we see are:

* Hostname was NOT found in DNS cache
*   Trying 155.98.20.215...

and

*   Trying 155.98.20.215...
* TCP_NODELAY set

Note these errors are coming from different machines. The first is when connecting from a mac and the second from either a linux box or even the localhost which is running the server.
