should generate:

- H1
    - H2a
        - H3a
    - H2b 
        - H3b

instead of:

1. H1
    1. H2a
        1. H3a
    2. H2b 
        1. H3b

maybe :TOC can generate numbered list (for backward compatibility) and :TOCList (or something better) can generate unnumbereed list.

It would be nice if there was an option to UpdateTOC, instead of adding a new one. Sure, it doesn't take much to just delete the TOC and create a new one, but it would be even faster if one could just UpdateTOC instead.

You cannot have some characters in the TOC links, it breaks the linking part. For example:

# This is a question?

Using your extension becomes:

1. [Is this a question?](#Is-this-a-question?)

That will not properly link you. You have to remove the question mark in the link part.

This is also true for "(", ")", etc..
