Target therapeutic levels for calcineurin inhibitors after transplant (liver)

• cyclosporine
  • 1st three months: 200-250 ng/mL
  • after 12 months : 80-120 ng/mL
• tacrolimus
  • 1st three months: 7-10 ng/mL
  • after 12 months : 4- 6 ng/mL

• Tacrolimus
  • Starting dose : 0.15-0.20 mg/kg/day - 2 divided doses
  • Ctrough 8-16 ng/mL (Whole blood)
• Cyclosporin
  • Starting dose : 8-10 mg/kg/day
  • Ctrough 200-400 ng/mL (Whole blood) - first 3 months
  • Ctrough 100-300 ng/mL (Whole blood) - thereafter
