It also looks like the tag is being added each time I push a tag. Is that suppose to happen?
Looking through the code, it looks like this is happening because since the 404 is being thrown it never successfully resolves, thus not setting loaded = true, so it keeps trying to add it. So the main issue is the main issue mentioned the 404 on the script file.

I just had the same issue. Your comment made me think a little more and I noticed I didn't deploy the GTM property yet.
Did this, and it works now.

In the Google Tag Manager UI, the submit button on the top right. I guess publish would be a better word for it.
