Class 2 patient and doctor is an m:n relationship. 
A separate table needs to be bulit

1. Partial key is to be represented by a - - - - dashed underline
2. 'Assigned Treatment' and 'Associated' relations are redundant as 'Recording Treatment' gives all the information that is needed.
3. Patient ID in payment and records need to be a separate attribute, it could either be an attribute of patient or person and you can create a foreign key.
4. Patient Entity is logical however in this scenario its redundant. You could use Person in place of Patient Entity and you would get the information required. (just the person id or patient Id).
