The Extension works well, with preview images of 16x16 (1st Image) and 128x128 (second image), but when the second image gets bigger (256x256) the image information goes past the 1000 lines, and thus no preview is shown.
Maybe in this case the first image could be shown or just increase the searched through lines (might be to slow).
But I don't know much about Shell Extensions.

You want to register a dll in the batch file, but you do not provide the dll. GcodeThumbnailExtension.dll does not exist in this zip file

not really an issue, just a request. Could you (please please) drop a built dll for your project : it would be very usefull 'cause I do not manage the %$! installation process for the right skd with the right msbuilt etc..
