Add TTLs to keys.

Adding TTLs that are checked on read would be pretty simple. Could also add a janior that cleans up old keys.

This should be added with documentation that indicates that short TTLs will remove most of the advantages of CAR over other caching algorithms.

Most caching algorithms are designed for situations where you are dealing with equal size pages (think, filesystem caches and database caches). While I don't know if it's appropriate for car_cache to try to balance the caches based on the size of items; if the max_size was adjustable on the fly, one could write a process that monitors the size of the cache and adjusts the max_size to stay within some memory bounds.

We need a way to manually delete keys from the cache, both the t1/t2 and b1/b2 tables.
