Hi @Nolan-Steed,
I have looked into it and this is the correct behaviour. According to the documentation for count_occurrences_by_grade (see here), each patient is counted once per their highest grade level. In the case you provided, one of the patients with a grade of 1 has the following 3 records. Since there is a record with grade 2 the patient is counted under grade 2 instead of grade 1.
