Until now, we assumed the passed ele contains a flat chain of block-eles.
In that case refillParagraph() doesn't work anymore on a page-break within an inline-ele.

Currently we're prepending the book-ele to the passed ele, better make passed ele be the book-ele, otherwise the passed ele cannot be the content's root-ele; the body-ele.

in reference to your tweet cited below I started a prototype here, which is by no means finished, but it should work for your requirements at the current state, in case you're interested in a JS-solution, which I'd choose for cross-browser-compatibility.
