No fall damage

Bug report from the community:

"you can fall any height without taking any shock damage. Is there anyway you can make it so we go unconscious from certain heights?"

Adding some options

enable/disable GRAD Undead
enable/disable death after amount of time beeing unconsious
enable/disable bleedout after a certain amount of time
enable/disable and configure fall damage
@nomisum mentioned: we could also add a setting that prevents instadeath but allows for the double tap to kill

All settings could be done in the default player manager

Unconsciousness in water

@nomisum reported: there is also an issue with uncon in water currently , you instantly fall uncon again after waking up with 1%
