Looks like I haven't given it enough thought. It was actually quite easy:

function moat(num) {
  let t = throat(num);

  return f => {
    return new Promise((resolve) => {
      t(() => {
        let promise = f();
        resolve({promise});
        return promise;
      });
    });
  };
}

Usage example:

let m = moat(5), results = [];
for await (let item of someStream) {
  let result = await m(() => process(item))
  results.push(result.promise);
}
let data = await Promise.all(results);
