As a user , I want to use form without auto-completion so that i can fill without having to delete some populated fields

As a user, I want see when adjustment was performed so that i can track.

I think there should be date as of when adjustment was performed and who did that action.

Describe the bug
When results are filtered if the results pages are less than the page user was on when filtering the number doesn't reset to 1.

Expected behavior
On filter the page number should reset to 1 as user start to paginate the returned results.
