Create the required directories for the output file

I use the "prepare-package" phase in the plugin, and the target directory is the ${project.build.directory}/webapp/cache.manifest

The plugin fails because the directory does not exist. In order to succeed, I added the following line:

    try {
---->   outputManifestFile.getParentFile().mkdirs();
        outputManifestFile.createNewFile(); // create it if it doesn't yet exist
    } catch (IOException e) {
        getLog().error("IOException creating manifest file: " + outputManifestFile.toString(), e);
        return;
    }

It would be nice to have an option like "forceCreateOutputDirectory" in the configuration.

Add support for external files

I'd like to be able to add something like

 ...
 <include>http://foo.com/fu.css</include>
...

so that external libraries can be loaded from CDN, which doesn't work at the moment --- the include statement is ignored.
