@everett-wetchler is this information, if provided, submitted to along with the approved incidents?

Are we planning on saving this information in any particular document or a new document?

Yes, it'll be submitted along with the approved incidents.

It would probably need to be saved as a new document (perhaps called something like AdditionalSubmissionInformation). Its fields should be 'ori', 'submission_year', and the three questions above.

Does the user only have the ability to create and not update these values?

Good question.

I think the values ought to be editable. I'm thinking that they could be shown along with the submitted incidents in the "Past Submissions" area, and there could be an Edit button there.

@everett-wetchler , what do you think?

Hmm I don't think editing submissions is a good idea. Maybe have them confirm ("Double check your entries. Are you sure you're ready to submit?")

Ah yeah, I agree with @everett-wetchler now. Since you can't edit incidents after they're submitted, it makes sense to not be able to edit these fields after submission either.
