Currently there are a lot of static accesses to DiscordBot.MainInstance. Instead we should be passing command context around.

Additionally, command context should probably include GuildOptions (and potentially cached versions of categories and issues as well) to avoid multiple DB reads

If two separate issues are created simultaneously, there is a possibility of a duplicate issue number due to DB access race conditions

Command skeletons already exist in IssuesSummaryModule.cs

(names not finalized, just for reference)

• note for appending text to issue descriptions
• attach for attaching images
  • other file types?
• linkmsg for adding a message link to an issue
  • maybe add functionality for replacing the default message link as well
• copydesc for copying an existing message into the description of an issue

Similar to #1 race conditions exist for vote filing. We should probably mitigate this by creating a database table for votes rather than incrementing a vote count on a proposal

Currently the github reaction does nothing
