Make StateFn callback take a mutable ref to output vec instead of returning a new Vec

It is very inefficient to allocate a new Vec on every state transition.

The existing definition for a StateFn is

pub struct StateFn<T: FsmTypes>(
    pub &'static str,
    pub fn(&mut T::Context, T::Msg) -> (StateFn<T>, Vec<T::Output>)
);

Change it so that it looks like this instead:

pub struct StateFn<T: FsmTypes>(
    pub &'static str,
    pub fn(&mut T::Context, T::Msg, &mut Vec<T::Output>) -> StateFn<T>
);

Also, need to fix tests and docs when this change is made.
