EDIT : I have resolved it by change this line:
return $matches[1] < 192 ? '<img src="pic/smilies/' . $matches[1] . '.gif" alt="[em' . $matches[1] . ']" />' : '[em' . $matches[1] . ']';

To this:
return $matches[1] < 384 ? '<img src="pic/smilies/' . $matches[1] . '.gif" alt="[em' . $matches[1] . ']" />' : '[em' . $matches[1] . ']';
