I'm unable to reproduce this.

Here's my test app:

  "os"
  "github.com/codegangsta/cli"
)

func main() {
  app := cli.NewApp()
  app.Name = "test"
  app.Usage = "verify help output"
    app.Commands = []cli.Command{
        {
            Name:      "hi",
            Usage:     "greet someone",
            Action: func(c *cli.Context) {
                println("Oh, hello there.")
            },
        },
    }
    app.Run(os.Args)
}

And here's the output that I get (on Darwin/amd64):
