Further research into this has revealed that the problem is related to whether or not the font in use contains a glyph for the unicode characters. If the font does not contain the glyph, Vim (or perhaps the system, which is why the behavior is the same in iTerm) will attempt to find a font that does.
This also explains why Menlo doesn't appear to have this problem while Bitstream Vera Sans Mono is exceedingly slow: Menlo contains 3,157 glyphs while BVSM supplies just 269. Courier, for reference, includes 1,343 (which seems like quite a few and still does not include the double arrow used for extends
).
