This is also an issue for me right now. It was working in spree 0.50.0 but after the upgrade no go. I haven't even looked into the reason why but my guess would be an change in core around the _address.html.erb file...

I'm at the Address page and I don't see any options for me to select previously entered addresses

Hi, you can select only addresses, which will be entered after installation of extension.

Romul - This still isn't working on spree 0.60.0. You never get the prompt to select the address.

I'm sorry, but I work with this extension and spree 0-60-stable more than 4 months and I can't confirm what you say.
It works fine with 0.60.0.rc, 0.60.0, 0.60.1 and current 0-60-stable for me. So I need at least more info about your environment, maybe you have some customizations in your own site or use some incompatible extensions..
