It appears that this happens due to incorrect decryption of sector data, resulting in garbled data.
Oddly enough, decryption of sector data seems to work very often, and when it doesn't work correctly, at least the decryption of sector positions works correctly.
The exact circumstances of why this happens will need to be investigated
from w3x-to-vmf.
