package.json is only generated when you run lein npm. The plugin hooks into lein deps and runs lein npm install automatically, but that's the only exception currently. Actually the lein npm start solution sounds like the better way to launch Node apps, so it might be good if lein run invoked that (and set up scripts.start automatically).
I don't have any spare time to work on this project currently, but I'd be happy to merge a patch if you want to take a crack at it. :)

Sure, I am short on free time myself and there's a learning curve involved for me here, but I will eventually get a pull request your way.
