blockquote feels like a stopgap approach to fulfilling the need of having a piece of text highlighted and applied to an article in whole. q accomplishes this while keeping an overall fluid feel to the reading experience. I have also never used a photo in blockquote.

left and/or right aligned hanging blockquote would serve the purpose of highlighting text within a particular paragraph (text that wouldn't necessarily be applicable to the article as a whole) without breaking reading continuity. The left and/or right aligned hanging photos would serve the same purpose.

I feel having the photos/text hanging (partially within the text block and partially outside of the text block) feels less distracting; like I'm reading "past" them, rather than "around" them.

I agree with @seanichol about not putting photos into a blockquote, I reserve it for text only. As you are likely aware, Github styles blockquote like Saga does. That certainly doesn't mean it is the best design for it, but I like to think Github has a pretty standard set of styles.

q looks quite nice and I would leave it as-is (larger font, italics, center-aligned).
