Err :connection error: desc = "transport: Error while dialing dial tcp 192.168.4.27:26257: connect: connection refused". Reconnecting...
tells me your worker nodes do not have iSCSI dataplane connectivity to the iSCSI targets on the FlashArray.
Please confirm you can ping the FlashArray iSCSI IP address from the worker nodes.

yes ping works just fine, and the connectivity worked prior to the upgrade.
