Hi,

libspatialite.5.dylib was the libspatialite 4.1.1 install, which gdal's install is still referencing. However, you have probably updated to libspatialite 4.2.0 by now (libspatialite.7.dylib). So, it is a gdal linking issue, and only symptomatic via QGIS's launch.

You can verify by running the following in Terminal: ogrinfo --formats | grep 'SQLite'. You should see the same link error.

Try:

brew reinstall gdal -dv

Upon successful rebuild, ogrinfo --formats | grep 'SQLite' should return -> "SQLite" (read/write).

Then, try relaunching QGIS again.
