I would prefer to explore this issue on the Arduino forum, although I know I posted the article on the gammon.com.au forum, and the code is here. However it is easier to keep the Arduino-related posts in one place.
If you create a thread there (probably in the Microcontrollers section) feel free to send me a PM via the forum to alert me to its existence.
The brief answer is that it hasn't entered programming mode, as those lines are shown before it does. Why not, is a mystery. I think I have the original Mega board, so I can't directly test it on the R2 or R3 board.
