We finally managed to determine what the issue was, however I still feel that a sample project would be very helpful, so I am leaving this issue open.
Our issue was a combination of not having an online test database available because MYOB removed it without clearing the information that said we had one, and the fact that our customer was not entering the information correctly since they were using the new SSO which we did not support at that time. Once we got a new online database to test with, we were able to determine the required setup and to successfully connect. We then got things straightened out with our client.
