I found the reason for this error. Looks like you HAVE to register the app in: https://apps.dev.microsoft.com NOT in the azure management portal. Totally a pain as all the references redirects to the azure portal.

I worked on some tests before and other APIs only require the app to be registered in azure portal:
https://portal.azure.com or https://manage.windowsazure.com, in this case you need to register in https://apps.dev.microsoft.com
I overlooked this link on the description as I though azure portal was the only registration point. Looked over the web and looks like a LOT of people had same issue so it was worth mention.

I've come across this issue, when trying to follow https://github.com/cbales/report-generator. Registering an app in the Azure Portal, or at https://apps.dev.microsoft.com made no difference for me, sadly.

Ah, solved: I used https://apps.dev.microsoft.com to register a 'Converged Application' rather than a 'Azure AD only application'.

Certainly it is hard to find out where to register the application and how to use it.
I would like to update the README and tutrial at a later date.

Though it may be still a little confusing UI, I changed some UI and tutorials, and released a new build.
