It seems like the reasonable thing to do would be to create a new migration that contains all missing foreign keys. Developers can then review it and make modifications before running it.

There would be a lot of gotchas to this, like ignoring polymorphic associations, situations where tables are used by multiple models, and determining how smart to be about reading association options. For example, should the :dependent option be considered? Same goes for :primary_key and :foreign_key.

Anyways, i do like the idea of generating a migration and allowing users to correct errors during development time.
