I would advise you to check with tagged release, as flashls/master has not be fully tested.
could you try to reproduce the first issue with hls_debug=true ?
for the second issue, the logs at the end are expected until the level 4 manifest is retrieved ... doesn't the player recover after some time ? or maybe the HTTP GET request is stuck ? have you checked the network chart ?
also if it is live playlist, you might try to turn hls_live_flushurlcache=true

Continues to play video and loading playlists fine.
There are no errors as such, it just recognises the audio track as audio1 instead of TS/AAC. Everytime this happens, no audio plays. However, it only happens some of the time. I suspect you may need a stream to identify this issue?

As you can see dropping the alternate audio descriptor solved it. However afaik the first method should still be valid.
