Would this ever be needed? Should not any offset be handled by server through the sync procedure?

If the automatic synchronization on the server side is limited to 30 minutes for instance and you'll shift the device time for instance for 2 hours, then this delta would be needed. Otherwise you have to use the manual synchronization.
But yes, it's the feature which majority of users will not use (it should be in advanced options section) :)

Perhaps not entirely useless feature, it seems openOTP had at some point a limit of 900 seconds - https://groups.google.com/d/topic/rcdevs-technical/y9Id63C-vKs/discussion
