try this to ensure the promise complete:

document.saveQ().done();

I'm already use many middlewares but I'll investigate it now.

That doesn't solve the issue. I include a code snippet to demonstrate this:

var UserSchema = mongoose.Schema({
    name: String,
    email: String,
    password: String
});
UserSchema.pre('save', function (next) {
    var user = this;
    console.log('modified: '+user.isModified('password'));
    next();
});
var Users = mongoose.model('Users', UserSchema);
