You found a bug. The variable error was undefined, hence the crash you experienced. I've changed sample.js so it now provides an error in the response instead ('Authentication failed').
There are no requirements for which status codes you should use on the relying party (i.e. client) side when authentication fails. Choose whatever is appropriate for your scenario. 500 is just a (bad) choice made for our sample.
