I think we touched upon this one elsewhere - the way to do this is to use either f:uri.resource or v:extension.path.resources (depending on wether or not you need BE support; in this case you should use v:extension.path.resources).

The problem with extRelPath translation is that unfortunately the relative paths are invalid when rendered in the "new content element" wizard, the php file of which is located in the cms extension. As a curiosity I can add that the new content element wizard itself does not support the EXT:... file reference syntax. It should :)

Anywho: using ViewHelpers here will ease your worries and make you a very happy man - given that you can also set image widths etc. and have TYPO3 generate scaled versions :)
