Php proxy fails when used with secure web maps about resource-proxy HOT 10 CLOSED

@kellyhutchins @jgravois It's not clear to me how the .NET one works.

@kellyhutchins would you please test the OAuth workflow for the same web map you are using? I know you said it fails using Username and Password, but what about the OAuth flow.

On my test machine I have the PHP proxy authenticating and it will show web maps using Username and Password flow, but the operational layers don't show and I am starting to think it has to do with with exchangeToken logic.

@jgravois Would you test .NET with this web map below... and confirm .NET shows both basemap and operational layer?

i've already tested this in .NET and it definitely works. we get a token using "./sharing/generateToken". @phpmaps if you wanna dig into this deeper, i'd be happy to.
