Output of launch configuration shows the EFS volume cannot be mounted:
+ mkdir -p /mnt/efs/
+ chown ec2-user:ec2-user /mnt/efs/
+ mount -t nfs -o nfsvers=4.1,rsize=1048576,wsize=1048576,hard,timeo=600,retrans=2 fs-********.efs.us-west-2.amazonaws.com:/ /mnt/efs/
mount.nfs: Failed to resolve server fs-********.efs.us-west-2.amazonaws.com: Name or service not known
The directories in /mnt/efs/ do exist, so resolving the mount should fix the issue.

After adding the mount targets, the website recovers after instance termination.
