At first I didn't read the last sentence. Was that meant to be the first chapter in the story? Chapters are ordered by published date, not by creation date (which I think makes sense; you might start a draft and then realise you need a chapter to come before that). Could that be the confusion/disagreement?

I can reproduce the other bug, I'll fix. Thanks a lot!

I was thinking the chapters went from most recent to least recent. That's a good schema -- the only problem is that if you go back to edit an earlier chapter (or fix a typo, etc) it shows up out of chronological order. Best way to clarify would be adding chapter numbers -- a recommendation I will make in the UX document.

Saving a chapter (to fix typos) shouldn't change the publishing date, so that shouldn't become a problem. Unless you press "Publish" instead of "Save" (and thanks to this conversation I realise that having a "Publish" button when it's already published is probably stupid/confusing).

Chapter numbers are probably going to be very tricky, at least if we show them to the players (because not all characters participant in all chapters). But we can talk about it via e-mail or whatever.

The main issue (duplicate chapters) should be fixed now. Let's talk about the ordering issue separately.
