Hi,
Thanks for your report - I'm incredibly excited that you've raised the first issue for this project!.
You found a bug: the 'checked' variable was not declared. I've fixed this in the source code. Until the new version is released, you can install directly from the source:
pip install git+https://github.com/blasferna/django-crispy-formset-modal
Please let me know if you encounter any other issues.

I am very happy to be able to contribute to this project which I consider really interesting to solve the problem of managing formsets in Django.
The last change works great!
I'm going to try different types of widgets, I'll keep you updated.
