That's usually when an ID is used on the model that is not part of the embedding matrix, so it's weird it would show this error if you correctly reshaped the model. Make sure that the model is not using any cached dataset data that may have a tokenized version of the text with incorrect IDs, or try to do a test run without cuda to have a more informative error. But my bet is that the model is receiving some unexpected token ID.

I'm still diagnosing the reason for detecting unexpected token IDs. I'd probably have to adjust some code to account for custom training data with different entity and relation numbers. I will try keeping you posted, but if you have any suggestions, please let me know!
