If the error is ActionController::RoutingError, Rails has not passed the request to the controller, since it can't determine which controller it is. Without knowing which action it's going to run, Rails can't determine which filters to run before the action, and doesn't run any of them: vanity_context_filter never gets executed.
What about calling the filter directly from rescue_from?

I'm no longer working on the app where I was experiencing this, and I'm not using Vanity in anything currently. So if no other users are experiencing this issue, this ticket can be closed.
