I aggree that having a feature like find-it-or-add-it would be great. There is what the system knows and what the user can contribute to. Having one widget to handle both of them would be perfect.

Yes that's it. The options are suggestions.

Yep you got it! As ymacon says the drop down would basically just be suggestions so no matter if there is a match or not the item would be added.

+1 ... in multi select mode, this would make it a cool tag list widget, where you can add custom tags on the fly.
